Testimonial—If CoBRA® has helped you in a planning, training, exercise or real-world situation, please share your testimonial, story or photograph with us and others. To thank you for sharing your story, we will send you a CoBRA® gift in the mail.
For your submission, please include the following:
- Your testimonial, case study and/or photograph. We are interested in your testimonial about CoBRA®, your story about how CoBRA® has helped you do your job, and a photographs of you—in your uniform or work attire—using your CoBRA® system(s).
- Your completed and signed permission form. We would like to share your story or photograph with others and highlight it on our Web site. All submissions must be accompanied by a completed and signed original permission form (please do not send a copy). Download and print a permission form. This form must be signed by the chief department head, public affairs officer, or equivalent.
- Your contact information. Please include your business card or a paper with your name, title, organization, department, phone number, email and mailing address so that we can accurately display your submission and to ensure you receive your CoBRA® gift.
